The accurate collection of the patient’s medical history and current medical condition is vital to a successful surgical outcome.
Using the Vigilant Preoperative Evaluation, this information is captured into the system and made readily available to all clinicians across the entire surgical continuum of care, ensuring consistency and accuracy throughout.
Patient satisfaction and perception of the professionalism of the facility is improved as a result of this complete evaluation providing their health history only once. Patient safety is enhanced due to more accurate and consistent information made available to all clinicians.
Further, the collection of accurate patient information in advance significantly reduces delays and/or cancellations the day of surgery while minimizing the cost of unnecessary pre-admission testing and duplicated procedures.
Patient History and Physical
This is the initial step in preparation for surgery. The system guides the user through a logical yet comprehensive series of screens for capturing the data. All data is stored within the Vigilant central database for access, reporting and queries. In addition to patient medial history, nursing intake data is recorded. Subsequent pre-op evaluation visits can be pre-populated with data from previous visits, which eliminates lengthy repetition of data entry.
Vigilant Pre! provides multiple levels of data capture including:
• Patient self-entry
• Telephone pre-admission support
• Clinical visitation
• Streamlined day of surgery preoperative evaluation
• In-house day of surgery preoperative evaluation
The primary output is an immediately web-available, complete, legible PDF-formatted report. Following the medically expected format, the report can be accessed through a web link, or directed to the hospital’s EMR and accessed as part of the patient’s electronic medical record. Clinicians can access this report from any networked computer, providing instant availability of critical medical history and preoperative evaluation.
